No no no no no NO, I was using that old cliche as a jokey reference to unexpected trauma in general. I can only imagine the horror at finding yourself face down in thick soupy mud with a broken neck, only to have your onlookers laughing and prancing and splashing you with yet more mud, but totally cool with leaving you face down in the mud until you choke.

I hate to spoil your Japan bashing, but the dude's fine. It may have been painful, but obviously not serious if he was shaking his head. That, and he didn't act like he was in pain. His face at the end expressed more of a "Yuck! Mud!" emotion than a "Jesus fuck! My neck!" emotion.
