Gender, but not sex. I believe that is why Gmork is upset about trans-individuals: in his eyes, their socially constructed gender clearly does not match their biologically defined sex. He finds the presentation dissonant and unnerving.

His statement is a little clunky, but becomes clearer if you understand that when he says "someone is actually", by the word "actually" he refers not to gender but to the individual's sex. (i.e. the subject may be gendered female, but "is actually" male)

You're right O_Z. I've seen people of all genders that experience body dysmorphic disorder after a period of physical change, like losing a lot of weight or gaining it and so forth. It happens to almost everyone during puberty, to a small degree. And I can't think of any bigger change than that brought about by hormone therapy and transitioning.
Maybe it's that once you start really focusing on your physical form and seeing it as something that can be changed, it is easy to alter your perception of what that change should look like.

The first trans person I actually knew well (who chose not to transition for both personal and financial reasons, although we've been out of touch for years so who knows now) went through a period of a few years after figuring out what was up during which she felt so alienated from her own body that she put on a ton of weight and rarely bathed because it was hard to do something that intimate to the body that no longer felt like her own.

Which makes perfect sense to me.
